The SBGCD Dental Care Plan
A membership run by this office for patients without dental insurance. You pay once a year, your first exam and x-rays are free, and everything else is discounted.
Individual
$350 / yearOne person, one year.
Dual
$325 / yearPer person. Parent and child, or two partners.
Family
$325 / yearPer person. Add as many members as you need.

What the plan does not cover
Worth reading properly. We would rather you knew the limits before you joined than after.
- It is not dental insurance, and it is secondary to any dental plan you already have.
- It cannot be used at any other dental office.
- It cannot be combined with another dental plan.
- It does not cover injuries under Workers' Compensation, or care covered by auto or medical insurance.
- It does not cover referrals to outside specialists, or any hospital charges.
- Membership is non-refundable, and a lapsed plan costs $50 to reinstate.
- Your portion of the bill is due on the day of service.
- The plan is reviewed and may be revised each year.
If you already have insurance
Then you don’t need this. We submit claims on your behalf so you get the full benefit you have already paid for. Call and we’ll check your coverage before your first visit.
